Colorado Coalition for the HomelessThe Colorado Coalition for the Homeless has been helping the homeless for more than 30 years. They consider the definition of homelessness to be anyone who lacks a fixed, regular, or adequate residence. More than 3.5 million Americans, including more than 1 million children, will seek lasting solutions to homelessness in 2015. Each year, the Coalition moves Coloradans into safe homes, keeps families together, and provides critical health care and support services for people experiencing homelessness. They provide more than 13,000 men, women, and children with medical and mental health care. Each night, more than 2,300 families and individuals have a place to call home in Coalition housing. In September 2014, the new Stout Street Health Center and Renaissance Stout Street Lofts opened. This new development increased integrated health care access for up to 18,000 homeless individuals each year and provides supportive housing for 78 formerly homeless households. Watch the video. A Kaiser Permanente investment is to create replicable integrated care teams including behavioral health that improve patient outcomes, improve service delivery, and cost effectiveness.
