Advance Care Planning - ConversationsAdvance care planning is a process of reflection and communication, a time for you to reflect on your values and wishes, and to let others know your future health and personal care preferences in the event that you become incapable of consenting to or refusing treatment or other care.
Advance care planning means having discussions with family and friends, especially your Substitute Decision Maker – the person who will speak for you when you cannot. It may also include writing down your wishes, and may even involve talking with healthcare providers and financial and legal professionals.
You may never need your advance care plan – but if you do, you’ll be glad that it’s there and that you have had these conversations, to make sure that your voice is heard when you cannot speak for yourself.
Five Steps of Advance Care PlanningToo many Canadians don't have a plan for their future healthcare. But Advance Care Planning is easy! There are just five steps.

Advance Care PlanningIn times of uncertainty, a little piece of mind can go a long way. Having an advanced health care directive in place can help guide decisions around how to care for you or a loved one if you or they are unable to communicate.
In other words, advance care planning puts you in the driver’s seat of your own health care journey, and helps avoid uninformed decisions by "back seat drivers".
An informed health care agent will understand your health care wishes, including what medical treatments should be continued or discontinued, should you be injured, severely ill or reach a point of functional decline when you can no longer speak for yourself.
This seminar, presented by Sharp HealthCare and the City of San Diego, provides information about understanding and documenting your health care wishes.
